• Social Security Number (United States Residents)
• Two witnesses who can attend in person or by video. They must be over 18 with valid government issued ID. If you do not have witnesses they can be requested for $25 per person once this purchase is completed.
• Utah marriage application fee of $71.75 which is not included in the cost of this purchase.
